Pre-K Soccer Registration (Spring 2006)

Fill out the information below, check it carefully, then click "Submit" at the bottom of the screen. A "disclaimer" page will open up. Please print three copies of the disclaimer, saving a fourth copy for your records. Repeat the entire process if you are registering more than one player. If you make a mistake, you may register a second time. To complete the registration process, send the three copies of the disclaimer, a copy of your child's birth certification, and a check for $65 to the address indicated on the disclaimer page. The check should be made out to "Irvington AYSO." You will receive an email confirmation of your registration.

Please note:  Your registration is not complete until three copies of the signed disclaimer form with your child's information, a copy of your child's birth certificate and a check or paypal receipt have been received by the registrar.  The mailing address is:
